In this Impact Assessment we consider the follow-up to the recommendations we had made in our 2006 audit entitled Care Coordination and to the undertakings given at the time by the Minister of Health, Welfare and Sport (VWS). Our recommendations had related to the problems we had found in the providers' coordination of care, the role of care insurers and the way in which the chronically ill experienced the care they received. This Impact Assessment also outlines the current state of care coordination for patients with chronic ailments (such as diabetes, depression, heart disease and neck and back pains).

Still not enough attention paid to underage young homeless. Virtually all 43 central municipalities in the Netherlands consider young homeless people aged between 18 and 25 in their problem analyses. The quality of the problem analyses, however, must be improved. Strikingly, only a limited number of central municipalities include young homeless people below the age of 18 in their problem analyses.

The Court of Audit has investigated the policy on the transition from management by supply to management by demand in the care and education sectors. Management by demand gives greater priority to the needs of the service users. This form of management is still in its infancy, which is why our audit was exploratory in nature.
